You may obtain registration forms at the following locations in Davie County.

• Board of Elections Office,161 Poplar Street (Monday through Friday, 8:00 a.m. through 5:00 p.m.)
• At either of the public libraries during regular hours
• At various public locations such as the Register of Deeds office, Tax office, Chamber of Commerce, Bermuda Run, Cooleemee, and Mocksville Town Halls
• Department of Motor Vehicles when applying for a drivers license
• Guidance office at Davie High
• Employment Security Commission
• Various Agencies such as Social Services, Health Dept., Agencies dealing with people with Disabilities, Armed Forces Recruitment offices etc.

These forms may be mailed or brought to the elections office. In addition, you may make any change or correction by filling out the back of your voter registration card and returning it to the Board of Elections office.

IF MAILED, REMEMBER YOUR CARD MUST BE POSTMARKED NO LESS THAN 25 DAYS BEFORE AN ELECTION OR RECEIVED IN THE BOARD OF ELECTIONS OFFICE BY 5:00 PM, 25 DAYS BEFORE ELECTION.
